Wound consulting & management
Our Registered Nurse specialises in wound management, providing assessment, treatment, and ongoing monitoring of complex and chronic wounds.
Wound consulting can be provided in the home, community settings, or residential environments.
We complete a thorough wound assessment to understand the type, cause, and current stage of healing. This includes reviewing skin integrity, risk factors, and overall health considerations. From this, we develop a clear and practical wound care plan.
We provide hands-on management of acute, chronic, and complex wounds to support safe and effective healing. Care is tailored to the individual and adjusted as healing progresses. Our focus is on reducing complications, discomfort, and infection risk.
We assess risk factors for pressure injuries and skin breakdown and put proactive strategies in place. This may include positioning advice, equipment recommendations, and skin care routines. Prevention planning is designed to reduce future wounds and promote long-term skin health.
We support wound care following surgery to promote healing and reduce the risk of infection. This includes dressing management, monitoring recovery, and identifying early signs of complications. Care is provided in line with surgical and medical recommendations.
We provide education and guidance on wound care techniques, dressing use, and skin management. This support is tailored to participants, families, and support workers to build confidence and consistency. Education is practical, easy to follow, and suited to everyday routines.
We regularly monitor wound healing progress and adjust care as needed. This allows us to respond early to changes and ensure treatment remains effective. Ongoing monitoring supports safer outcomes and continuity of care.
We work closely with GPs, specialists, and other health professionals involved in the person’s care. Clear communication ensures wound management aligns with broader health needs. This collaborative approach supports coordinated and effective care.
